Follow these steps to complete the complaint form and submit your complaint to us.
Type your responses if possible. If you complete the form by hand, please make sure your handwriting and signature are easy to read. If they are not, your complaint will be denied.

Download the complaint form (PDF) from our website.
If you cannot access it online, some Superior Court clerk's offices may also have copies.
Before you begin, review the complaint guidelines. They explain how to complete the form, what information to include, how to organize supporting documents, and other filing requirements.
After you review the guidelines, complete each section of the complaint form using the steps below.
Complete Section 1: Your information
Enter your:
- Full name
- Date of birth
- Mailing address
- Daytime and evening phone numbers
Complete Section 2: Judicial official
Enter the full name of the judicial official.
Select whether your complaint is against a:
- Judge
- Family support magistrate
- administrative law judges
Use a separate complaint form for each judicial official.
Complete Section 3: Statement of facts
Describe the conduct you believe is judicial misconduct.
Include facts that help explain your complaint, such as:
- Names
- Dates
- Places
- Addresses
- Phone numbers
- What happened
If you need more space, attach 8½ × 11-inch pages. Number each page. Write on only one side of each page.
Add supporting documents
Attach copies of documents or transcripts that support your complaint.
Include only documents that relate to your complaint.
Complete the court case information
If your complaint is about a court case, provide:
- Case name
- Docket number
- Type of case
- Your relationship to the case
Add attorney information
If you or the other party had an attorney when the alleged misconduct happened, provide the attorney's:
- Name
- Address
- Phone number
Review your complaint
Before you submit your complaint:
- Make sure every section is complete.
- Make sure all handwriting is easy to read.
- Check that all information is correct.
- Keep a copy of your complaint and supporting documents for your records.
Sign before a notary
Do not sign the complaint form until you are with a notary public, commissioner of the Superior Court, or justice of the peace.
You must sign your complaint under oath before a notary public, commissioner of the Superior Court, or justice of the peace before you submit it. Complaints that are not signed in front of a notary will be denied.
Mail your complaint
Submit your signed and notarized complaint by mail only.
